Objective: Present a case of auricular arteriovenous malformation in a
30-yr-old male patient

Result: Patient had a pulsatile right auricular mass measuring
9.5x3.2x8.4cm. CT angiography identified a complex vascular lesion
supplied by ipsilateral postauricular, internal maxillary and
superficial temporal arteries. Preoperative embolization was done and
subsequently, a total auriculectomy with preservation of the bony
external auditory canal was performed, followed by reconstruction using
a cervicopectoral flap and split-thickness skin graft.An improvised
silicone tube was utilized to maintain patency of auditory canal. Post
operatively,flap remained viable, but the skin graft failed to
integrate.Patient reported resolution of ear pain and no change in
preoperative hearing.8 months postoperative, no signs of
recurrence.Patient was referred for prosthetic right ear.
Conclusion: Auricular arteriovenous malformation is a rare and notably
challenging condition. While history and physical examination provide
diagnosis, imaging determine the lesion's extent and contribute to
surgical planning. |
