Gossypiboma is a foreign body granuloma due to a retained surgical sponge. We present a case
series of two patients after rhinoplasty.
Both patients were referred to our clinic with persistent nasal obstruction and cacosmia
following rhinoplasty. In the former patient, the history suggested possibility of retained surgical
material in the nasal septum due to the surgical history. In the latter patient, a foreign body was
not suspected due to no sign of inflammation. Endoscopy showed nasal gauze with perforation
in one patient and a septal bulge in another. A preoperative CT PNS scan showed no
abnormality. Endoscopic removal of the surgical sponge was planned for the former patient, and
septal body radiofrequency volume reduction for the latter. Under general anaesthesia,
endoscopic removal of the foreign body was done in the first patient. In the second patient,
during the radiofrequency treatment, the presence of a foreign body was noticed on the septal
body and removed. Regular follow-up showed no complications with symptomatic relief.
Conclusively, proper history, clinical and radiological examination, appropriate sponge usage
and counting with surgical checklists are crucial.
