¸ñÀû: Posterior nasal nerve neurolysis has been proposed as a potential
chronic rhinitis surgery. However, contact ablation of the posterior
nasal nerve had been reported as a risk of 2.4% posterior nasal
bleeding at 20 days post-operation. We hypothesized that a minimally
invasive approach combining radiofrequency ablation of inferior
turbinates and laser posterior nasal nerve neurolysis (RPN3) could
obtain comparable results with fewer complications. ¹æ¹ý:A retrospective cohort study was conducted on chronic rhinitis who
demonstrated a poor response to medication. Patients with a total 24-
hour reflective total nasal symptom score (rTNSS) of 5, rhinorrhea score
of 2, and congestion score of 2 were included. The primary endpoint was
the change from baseline in 24-h reflective total nasal symptom score
(rTNSS), nasal obstruction symptom evaluation scores (NOSE), and the
response rate defined as rTNSS improved by 30% during 6-month follow-
ups. °á°ú:A total of 54 patients were included with a mean baseline rTNSS and NOSE
of 8.7(95% confidence interval CI, 9.2-8.2) and 61.57(95% CI, 66.3-
56.8), respectively. At 6 months, the mean change in rTNSS was -7.3(95%
CI, -6.6 to -8.0, p<0.001) and -57.8(95% CI, -52.7 to -62.9), with 83.2%
and 95.3% improvement, respectively, and the responder rates were 100%
for rTNSS and NOSE. No serious adverse effects, including posterior
nasal epistaxis, were reported through follow-up. °á·Ð:The proposed posterior nasal nerve neurolysis with combined laser and
radiofrequency posterior turbinoplasty alleviated rhinitis symptoms,
demonstrating a higher response rate with fewer posterior nasal bleeding
complications. |
