Results of Facial Nerve Schwannoma Treatment

¸ñÀû: Although facial nerve schwannoma is rare benign tumor that originates along the facial nerve in temporal bone, appropriate treatment is necessary because of the significant morbidity associated with invasion to surrounding structures and occurence of facial palsy. In this study, we analyze clinical data and treatment outcomes of patients with facial nerve schwannomas to introduce strategies for treating tumors. ¹æ¹ý:We reviewed retrospectively eight patients who were diagnosed with facial nerve schwannomas at Asan Medical Center from 2011 to 2014. °á°ú:This study included 4 males and 4 females, and the tumor site were 2 in right, 6 in left. Two patients with facial palsy of House-Brackmann grade IV or worse at the time of diagnosis underwent tumor removal via translabyrinthine approach and facial nerve reconstruction with sural nerve graft and hypoglossal facial nerve crossover. Of five patients with facial palsy of House-Brackmann grade I at the time of diagnosis, four patients had observation and one patient underwent partial tumor removal via translabyrinthine approach because tumor extended to cerebellopontine angle and there was no deterioration of facial nerve function postoperatively. One patient with facial palsy of House-Brackmann grade III had regular follow up.Five of eight patients had observation, and their tumors were located at labyrinthine and tympanic portions in two patients, tympanic and mastoid segments in two patients, and mastoid segment and stylomastoid foramen in one patient. The median follow up period was 1.8 years (range, 1-3 years), and during the follow up period no patient had worsened facial nerve function nor increase in tumor size. °á·Ð:According to this study, in the case of tumor localized in temporal bone and with preserved facial nerve function, short term follow up without any procedure may result favorable progress. We need further studies with long term follow up data.
