¸ñÀû: To evaluate usefulness of hearing aids (HA) verification with real ear measurements based on National Acoustic Laboratories for prescribing nonlinear gain in patients with SNHL. ¹æ¹ý:Total 81 hearing-aid users assessed with both functional gain test and real ear measurement (REM, Verifit, Audioscan) from 2014 to 2015 were enrolled. Satisfactory gain at each frequency (0.25, 0.5, 0.75, 1, 2, 2.5, 4, and 6 kHz) to the stimulation of soft (55 dB HL), moderate (65 dB HL), loud sound (75 dB HL)in REM was determined based on NAL-NL2. Word discrimination scores at 65-dB HL were evaluated. Functional gain of HA was also performed routinely with speech discrimination at 65 dB HL to evaluate HA. The tests were repeated at initial appointment, 1, 3, and 6-month follow-up. °á°ú:At initial REM, satisfactory gain was not obtained at 7.1 frequencies to soft, moderate, and loud sound stimulation. Mean improvement of discrimination score at 65 dB after hearing aid application was 21.1%. At 1-month follow-up, average number of unsatisfied frequencies decreased to 6.6 and discrimination score increased to 22.9%, which was higher compared to those of initial test (P < 0.05). Among 28 patients who underwent more than 2 times of REM, discrimination score showed significant improvement after 2 times of verification, mean 25.1 to 29.0%. Number of unsatisfactory frequencies on real ear measurements also showed marked decrease from average 8.7 to 6.0. Number of HAs satisfying NAL-NL2 method over 7 frequencies were 1, 4, 9 at initial on soft, moderate, loud sound respectively. At second follow-up, each number of HA increased to 6, 12, 13 showing statistical significance in soft and moderate sound (P < 0.05). °á·Ð:Patients' word discrimination scores showed significant improvement after HA fitting with real ear measurement. Repetitive HA fitting using functional gain and real ear measurement based on NAL-NL2 method would improve performance of hearing-aids. |
