Scarless Robot-assisted Sentinel Lymph Node Navigation Surgery via Retroauricular Approach with Indocyanin Green 'Firefly' Fluorescence Imaging in Clinically N0 Oral Tongue Cancer |

¸ñÀû: Recently, sentinel node mapping using indocyanin green (ICG) has drawn attention for its safety, high tissue penetration rate and praticability of intraoperative real time imaging. The main puropose is introduce our initial experiences of sentinel lymph node navigation surgery using ICG with the aid of robotic surgical system equipped with Firefly fluorescence imaging. We expect to gain maximal benefit of the ICG detection technique from our scarless retroauricular robot-assisted neck dissection technique. ¹æ¹ý:Three patients each diagnosed with oral tongue squamous cell carcinoma received robot-assisted surgery using Firefly fluorescence imaging in 2014. °á°ú:All cases presented borderline regional lymph nodes enlargement on preoperative imaging. 2ml ICG/normal saline mixture (0.625 mg/ml) was grossly injected at four points (superior, inferior, both lateral) surrounding the primary tumor the day before the operation. In all cases, partial glossectomy without primary reconstruction was performed via peroral approach. For the neck management, robot-assisted supraomohyoid neck dissection via retroauricular approach was conducted. ICG uptake was easily visible through Firefly imaging and the operations were uneventful and successful. In two cases, the final pathology of the sentinel lymph nodes sent together with lymph nodes of levels I-III were negative. In one case, the final pathology revealed 4 positive nodes out of 26. (sentinel nodes 4/11, I-III nodes 0/15) The false negative rate of the sentinel node mapping was 0. °á·Ð:Robot-assisted sentinel lymph node navigation surgery using ICG-Firefly fluorescence can be a feasible and a promising surgical technique for intraoperative planning of the neck management. |
