Prognosis of Conservative Management of Traumatic Facial Nerve Palsy with Different Onset Time in Temporal Bone Fracture

¸ñÀû: Facial palsy(FP) is mostly developed as Bell's palsy but up to 5% of FP are due to direct or indirect trauma. Many studies showed the prognosis of Bell's palsy and discussed surgical or medical treatment. But in traumatic FP, outcomes of surgical management were mainly studied. This study aimed to evaluate the prognosis of conservative management of traumatic facial palsy in temporal bone fracture and to compare the recovery rate of immediate and delayed onset.<i>¥ñ¥Ñ ¹æ¹ý:Between 1998 and 2014, total 37 cases of traumatic FP were retrospectively reviewed. Four cases were treated by surgical decompression and 6 cases were excluded due to incomplete data. Finally 27 cases were evaluated and these were managed with conservative treatment including high dose steroid and physical therapies. Surgical decompression was not performed because of patient's general condition. We analyzed characteristics of traumatic FP including causes, sex, age, onset time, type of fracture, CT findings, electroneurography and House-Brackmann(HB) grade. Also, we compared the recovery rate between immediate and delayed onset group. °á°ú:Initial HB grade showed 1 case in grade II, 9 cases in grade III, 9 cases in grade IV, 7 cases in grade V, and 2 cases in grade VI. Nine cases were in immediate onset group in which severe FP (HB grade IV,V,VI) was 7 cases. Eighteen cases were in delayed onset group and severe FP occurred in 10 cases. In overall, the rate of complete recovery was 40.7%. Conservative managed traumatic FP showed good recovery (HB grade I or II) in 22 cases(81.5%), but 1 case did not improved much better(HB grade IV). In immediate onset group, the rate of good recovery was 66.7%(6 of 9 cases). Delayed onset group showed that the rate of good recovery was 88.8%(16 of 18 cases)(p>0.05). °á·Ð:The results of this study demonstrated that traumatic FP with only conservative management recovered good facial function. There was no difference in prognosis between immediate and delayed onset of traumatic FP.
