Cholesterol granuloma is most commonly associated with middle ear disease. It is very rare in the paranasal sinuses. We present the first case of cholesterol granuloma of Haller's cell associated with exophthalmos and diplopia in a 55-year-old man. This finding highlights the importance of including cholesterol granuloma in the differential diagnosis of paranasal sinus expansile lesions with orbital involvement.
