Ocular Vestibular Evoked Myogenic Potential Testing for the Prognosis of Bells Palsy |

¸ñÀû: Several studies have indicated asymptomatic involvement of vestibular nerve in cases of idiopathic facial nerve palsy. The basis for such observation is the close proximity of the vestibular and facial nerves in the internal auditory canal (IAC). Some studies have even suggested that some of the superior and inferior vestibular nerve bundles may receive fibers from the facial nerve. The aim of this study was to investigate the correlation prognosis of bells palsy with involvement of vestibular organs through ocular vestibular evoked myogenic potential testing. ¹æ¹ý:A total of 104 consecutive patients with Bells palsy were enrolled. They were treated uniformly with steroid and an antiviral agent. All of patients underwent ocular VEMP(oVEMP) recordings within 1week after onset. and also underwent electroneurography(ENoG), electromyography(EMG), pure tone audiometry, calroic test and scaling with initial and final House-Blackmann grades. According to the final recovery state, patients were divided into two groups complete recovery group and an incomplete recovery group. The incidence of abnormal findings in each test was compared between the two groups °á°ú:Eighty-five patients recovered completely and nineteen patients recovered incompletely. The mean value of ocular vemp asymmetry was significantly higher in incomplete recovery group than complete recovery group(P<0.05). In the analysis for evaluating the factors acting on the prognosis of the Bells palsy, there was no association between the rate of ipsilesional PTA threshold, caloric test and rate of abnormal EMG and recovery state. But, the initial state of ENoG, initial H-B grades and the rate of oVEMP were significantly correlated with the rate of recovery(P=0.025, P=0.013, P=0.009). °á·Ð:The results indicate oVEMP could be used as a useful tool for predicting the prognosis of Bells palsy comparable to ENoG and H-B grades. |
