This report describes a case of with fibroma and columnar cell metaplasia of tympanic membrane in a 65-year-old female patient. The patient complained of progressive hearing loss on the left side developed 3 years ago. She had a history of otorrhea once 45 years ago, and had no tinnitus, otalgia, dizziness, previous otologic surgery, trauma history. Otoendoscopic examination revealed that the left tympanic membrane was thickened with whitish mass-like bulging without normal landmarks. Pure tone audiometry showed mixed hearing loss. Results of a temporal bone computed tomography (TBCT) showed a 7*4mm sized soft tissue density lesion filling in the medial aspect of the left external auditory canal, abutting the tympanic membrane. The mass was removed, and subjected to histopathologic assessment, which revealed extensive fibrosis with columnar cell metaplasia and subsiding inflammation at the tympanic membrane. This is rare case of intratympanic membrane fibroma which was surgically removed.
