A Case Report of Bisphosphonate-Related Osteonecrosis of the Jaw

Bisphosphonate reduced the activity of osteoclast result in suppression of bone resorption. Since 1960s, bisphosphonate was used in treatment of many bone metabolic disorders. After 2003, the necrosis of jaw was reported as long term side effect of bisphosphonate called as BRONS(bisphosphonate-related osteonecrosis of the jaw). 84-years old female visited in otorhinolaryngology clinic. She has both nasal obstruction and watery rhinorrhea since several months ago. In physical examination, mucosal swelling of right inferior meatus and accompanied pus collection were seen. She has history of long term use of bisphosphonate over 10 years for treatment of osteoporosis, since her right hip joint was fractured. In general anesthesia, we operated both endoscopic sinus surgery, debridement of necrotic bone, extraction of two molar teeth. After surgery, the patient follow up with our clinic and endocrinology clinic, stopped in bisphosphonate and continued with calcium/cholecalciferol combination. Until post-operation 1 month, there is no complication. We reported this case for BRONJ with maxillary sinusitis and post- operation management.
