Effect of Transtympanic Silicone Catheter Insertion for Chronic Patulous

¸ñÀû: Although several different surgical methods of treatment for
patulous Eustachian tube (PET) have been reported, standardized
treatment has not been established yet. The most commonly
reported technique were ET plugging. This technique appears to be
generally safe and effective, but middle ear effusions are
occurred as a complication. In this study, we designed silicone
catheter which resembles modified funnel shape (ID 0.6 mm, ED
1mm, length 2.5mm). The purpose of this study is to evaluate the
effectiveness of transtympanic insertion of silicone catheter
(SC) for chronic PET patients. ¹æ¹ý:Surgical treatment was performed in 19 of 32 patients.
Transtympanic insertion of SC was performed unilaterally in 15
patients. Five patients in 15 received simultaneous cartilage
tympanoplasty (CT) because of retracted or flaccid TM. Five
patients received CT only. The postoperative symptoms and
otoendoscopy were evaluated in all cases. °á°ú:Transtympanic insertion of SC was successful in all patients except two patients, because of narrow tympanic orifice of the ET. One patient was inserted partially but another patient was failed to insert. After insertion of the SC, the symptoms of PET were relieved. There was no incidence of postoperative middle ear effusion in all cases. However remained autophony in 2 of 5 CT only patients. One of them received additional interval insertion of SC. °á·Ð:Transtympanic insertion of SC provided a safe and effective for
reducing the PET symptoms. In case with retracted or flaccid TM,
simultaneous CT was successful. Further studies are necessary to
investigate the effectiveness of CT only in large number of
patients. |
