¸ñÀû: We aimed to develop a minimally invasive rabbit model of VX2
laryngeal cancer suitable for these two imaging modalities through
a transoral approach and to study the usefulness of combined OCT
and endolaryngeal EUS for evaluation of early and advanced
laryngeal cancers using this model. ¹æ¹ý:VX2 tumor suspension was inoculated into the vocal fold of 10
rabbits by injection through the trans-thyrohyoid membrane.
Correlation time-domain OCT and high-frequency EUS were used to
visualize laryngeal tumors in the area of protruding mass formation
in four rabbits at 1 week post-injection (group A) and in four
rabbits at 2 weeks post-injection (group B). °á°ú:Small submucosal tumors were observed with rigid laryngoscopy in the group A, and pathologic evaluation showed that the tumor close to the basement membrane of the vocal fold mucosa, but not invaded. The lining of the mucosa and basement membrane of the vocal fold were not broken and a tumor plug was detected at the protruding ridge by OCT. However, these lesions were not detected by EUS, and the overall shape of the tumor could not be clearly identified by EUS. A large tumor filling the laryngeal lumen was observed with rigid laryngoscopy in the group B, and the vocal fold was almost completely involved, including the paraglottic space, in pathology analysis. Determination of normal structure and tumor plug was difficult by OCT however, in EUS the overall shape and size of tumor infiltration deep in the paraglottic space was confirmed and the thyroid cartilage was shown to be intact. °á·Ð:This study is the first experimental trial assessing the value of
multimodal imaging using OCT and EUS in a laryngeal VX2 tumor
rabbit model. Combining OCT and EUS helps to identify changes in
whole laryngeal mucosa layer and could potentially identify
laryngeal tumors and predict how tumors progress. This combined
modality could help in viewing the tumor extent, assist in
diagnosis and help to establish a treatment plan for laryngeal
cancer. |
