A Branchial Cyst of the Pyriform Fossa Transoral Laser Resection

Background: Pyriform sinus malformations represent rare third and fourth branchial anomalies. They may be an unusual cause of dysphonia in pediatric. Case report: A 6 year-old girl presented with dysphonia since birth with no other complain. Clinical examination revealed midline cystic neck swelling 2x2 cm. Flexible laryngoscopy revealed left vocal fold (VF) immobility with a cystic swelling arising from medial wall of left pyriform fossa about 3x2 cm. Computer tomography revealed midline subcutaneous cystic swelling and submucosal cyst in the left pyriform. Transoral CO2 laser excision of the pyriform cyst was done with no communicating tract and surgical excision of the neck cyst. Histopathologic evaluation revealed pyriform branchial cyst lined by pseudostratified columnar ciliated epithelium and neck epidermoid cyst. The patient showed post operative improvement of dysphonia and the left VF mobility. Discussion: The present case is the first case to describe a Bailey's type IV branchial cyst in the pyriform fossa. It is the second branchial lesion transorally laser excised and the first third branchial cyst reported.
