¸ñÀû: To identify the causes of false result of bHIT, gain and corrective
saccade of video head impulse test (vHIT) were analyzed according
to the result of bHIT. ¹æ¹ý:vHIT positive 51 ears and bHIT positive 51 ears were included.
vHIT was performed using ICS impulse (Otometrics, Denmark), and
occurrence of corrective saccade and/or VOR gain below 0.8 was
decided as positive. Occurrence rate of corrective saccade was
calculated by dividing the number of corrective saccade, which
occurred during the repeated tests, by total repeated number of
test. The occurrence pattern and the occurrence rate of
corrective saccade on vHIT were compared between false result
group and true result group on bHIT. °á°ú:Eighteen ears among the total vHIT positive 51 ears showed bHIT negative, which the false negative rate of bHIT was 35.3%. Among bHIT false negative 18 ears, 2 ears (11.1%) showed only low gain without corrective saccade, 1 ear (5.6%) showed only covert saccade, and other 15 ears (83.3%) showed both covert and overt saccade. There was no case presenting overt saccade solely. This occurrence pattern of corrective saccade was similar in bHIT positive group also. The occurrence rate of overt saccade in bHIT negative group was 37.0625.97%, which was lower than those (82.718.37%) in bHIT positive group. When we categorized the ears according to the occurrence of covert saccade, occurrence rate of overt saccade in covert-positive group was 47.240.5%, while it was 81.928.5% in covert-negative group. Among total bHIT positive 51 ears, only corrective saccades occurred in 12 ears (23.5%) without abnormal gain which could be decided as false positive bHIT. °á·Ð:False negative result of bHIT is caused by low occurrence rate of
overt saccade which is influenced by the occurrence of covert
saccade. False positive bHIT can occur when overt saccades occur
even when the gain is normal on vHIT. |
