¸ñÀû: Evaluation of urgent tympanoplasty effectiveness. ¹æ¹ý:Were examined 83 patients aged 16 to 40 years,46 of them
women(52.8%), 41 men(47,2%)with acute traumatic rupture of the
tympanic membrane, caused by trauma of ear with hand. Remoteness
of injury before addressing to a specialist ranged from 1 to 12
days. Patients complained on sudden hearing loss-50%, pain and
noise in the ear with the release of a clear liquid-20%, also
observed patients with headache,nausea and vomiting of a
temporary nature.All performed otomicroscopy, audiological
research and CT-of temporal bone. °á°ú:Otomicroscopy revealed in 87 patients defect of tympani membrane with jagged edges in tense part of tympani membrane. In 16(22.2%) patients defect of tympani membrane was in anterior and posterior quadrant of tympani membrane, but violation of the integrity of ossicular chain was not detected. Audiological examination revealed in 69(95.8%) patients conductive hearing loss, 18(25.0%)sensorineural hearing loss (high threshold of ultrasound to 10 volts, violations of intelligibility to 80% and positive recruitment). On admission day, all patients underwent urgent tympanoplasty. During the operation, blood clots and nonviable torn edges of the defect of tympani membrane were removed. Re visioned of the tympani cavity, auditory ossicles chain in presence of liquorrhea myringoplasty performed by the temporal muscle fascia. Then prepared a place for laying the new eardrum. The postoperative course was satisfactory in the majority of patients, 50% showed positive morphological and functional results. Follow-up for 6 months - in 18 (20.6%) patients showed hearing loss and tinnitus. Repeated audiological examination revealed in these patients sensorineural hearing loss. °á·Ð:Thus, emergency tympanoplasty is an effective treatment of acute
traumatic rupture of tympani membrane with liquorrhea, the results
are convincing and can achieve positive morphological and
functional results in 80% of operated patients. |
