Objective: Inverted papilloma of the nose and paranasal sinuses is benign epithelial tumor with high rate of recurrence, local aggressiveness, and malignant transformation. For these reasons, inverted papilloma has been treated like malignant tumors with extensive surgical resection. With the help of endoscopic sinus surgery technique, it is now available to treat inverted papilloma with multiple stage surgery without severe complications which usually resulted from extensive resection. Methods: We report a successful multi-staged surgery case of inverted papilloma which invaded the lacrimal sac, the periorbita, the ethmoid and frontal sinus. Results: Three stage surgeries were performed including debulking surgery under general anesthesia, and removal of recurred lesion around lacrimal sac under local anesthesia. Control was carried out after surgery that showed no signs of disease recurrence after 3 years. Conclusion: Using multi-stage endoscopic sinus surgery, inverted papilloma which invades lacrimal sac and periorbita can be treated without severe complication with good aesthetic results.
